Applicant Checklist

 

This  document  will  assist  you  through  the  application  process.  Applicants  are  also  encouraged  to  visit

www.ccms.edu >  Admissions for general information.

CONSIDERATIONS BEFORE APPLYING

☐ Review your progress in completing prerequisite requirements

 Complete regionally accredited general education transfer credits.

☐ Know your state licensing requirements

Review the educational and licensing requirements of the state board in which you intend to obtain licensure. Visit http://nfda.org/licensing-boards-and-requirements.html.

☐  Register with your state board if applicable

Contact the state board in which you intend to obtain licensure for requirements in registering prior to beginning your studies at CCMS. States requiring registration are identified in the following resource: http://nfda.org/licensing-boards-and-requirements.html.

☐   English Language Requirement:

Applicants whose first language is not English must submit the TOEFL found at  http://www.ets.org/toefl. Applicants who have attended a school where the primary language of instruction is English may have this requirement waived.

☐  Have your official transcript(s) sent to CCMS

Applicants should request official transcripts from the Registrar of all colleges previously attended. There is a section in the Application to document from which institutions you have earned credits and requested transcripts.   Your application will not be considered complete until CCMS has received your official transcript(s). Official transcripts are to be issued by the college(s) Registrar directly to the CCMS Admissions Director in a sealed envelope or by electronic transmission.

 ☐   Gather items you’ll need to complete the application

When filling out the online admission, you will need to have several items at hand:

          ☐  Driver’s license OR government-issued photo ID

          ☐  Credit hours or degree earned for previous colleges/universities

In addition, you will have the option to upload electronic copies of several documents. If you do not have an electronic copy of the following documents, you will need to provide a physical copy before Orientation:

          ☐  Driver’s license/government-issued photo ID or recent photo

          ☐  Name change documentation (if applicable)

          ☐  Disability accommodation documentation (if applicable)

          ☐   Veterans – DD214 form (if applicable)

APPLYING

Online admission

Find our online admission process at https://www.ccms.edu/admissions/application-process/apply-to-ccms/. Effective 11-17- 16, there is no charge to apply.

Paper application

Print the application at https://www.ccms.edu/admissions/application-process/apply-to-ccms/. Effective 11-17-16, there is no charge to apply.

AFTER APPLYING

Upon receipt of your completed Application/Audit Request and receipt of all official transcripts, a determination of eligibility will be made within two weeks.

A. Acceptance (applicants who have completed all entrance requirements)

The following will be enclosed in our written correspondence:

 Letter of acceptance
 Student Enrollment Guide containing:

       Important new student information and policies

 Forms you will complete and return
 (Optional) Lab Assistant application

B. Provisional Acceptance (applicants who have completed most, but not all, entrance requirements)

The following will be enclosed in our written correspondence:

 Letter of provisional acceptance
 List of requirements you still need to complete

 Student Enrollment Guide containing:

       Important new student information and policies

 Forms you will complete and return
 (Optional) Lab Assistant application

It is the applicant’s responsibility to stay in contact with us. Once you have completed all requirements, you will need to submit evidence of this to the Admissions Director.

C. Ineligibility for Admission to CCMS

Ineligible applicants have not successfully met entrance requirements. You are encouraged to contact the CCMS Admissions Director to determine whether successful completion of entrance requirements is a future possibility. AFTER ACCEPTANCE TO CCMS

 Complete and submit forms

Return completed forms from the Student Enrollment Guide before orientation.

The Admitted Students section of our website contains everything you need to know to prepare for orientation and the first day of classes, including information on textbooks, registering for semester I classes, dates and deadlines, employment and housing, financial aid, class schedules, and email.

 Stay in touch

  If you have been provisionally accepted to CCMS (see B, above), you are required to               stay in contact with the Admissions Director to ensure completion of all requirements                 prior to orientation.
